Step One: Please contact us to discuss placement for your child. We will do our best to answer your questions. Our admission department may be contacted toll free at (800) 333-4009 extension 109, or via our e-mail contact form.

Step Two:: After talking with Admissions, we will send you a comprehensive application packet and require that you contact four former parents from the reference list we will provide. The application and calls must be completed to start the formal acceptance process. We need the Parent Application, Social History Questionnaire, and the Problems Checklist returned to us by fax at 765-662-1407 / Attention: Admissions, or by mail to:

Step Three: If your child is accepted, you will need to return provided Enrollment Packet.

Step Four: You can download additional information about the specific program in which your child is enrolled.
